A worker suffered fatal injuries after he plummeted nearly 30 ft through a skylight on a site in Cwmbran.
Daniel Dennis, who came from the South Wales town of Bridgend, died as a result of the fall, which occurred in 2003.
According to the Health and Safety Executive (HSE), the 17-year-old had received no training or induction to working at height prior to the accident.
Company owner Roy Clark was jailed for ten months after admitting a manslaughter charge in relation to Mr Dennis' death. He was also ordered to pay almost £18,000.
"This tragic case demonstrates the need for employers to take serious steps to ensure anyone working at height is properly trained and given appropriate equipment to do their job safely," stated HSE inspector Dean Baker.
He added that falls from height "very often" result in serious injury and death.
